Registered Nurse
The registered nurse is expected provide direction to members of the patient care team through case management of a patient. Case management of patient services includes:
• Perform initial evaluation, follow up visit and discharge of your patient.
• Establish the patient’s individualized treatment (Plan of Care) and medical record baseline.
• Collaborate with physicians, other agency staff and contract services to coordinate and implement the Plan of Care.
• Trains new clinicians by allowing clinicians to shadow patient visits.
• Documents direct patient services, coordination and collaboration with physician and other disciplines.
• Promotes the agency and services to the public and participate in physician and community education related to services provided by the agency as registered nurse.

Qualification
Required:
• Current California Registered Nurse license
• Able to travel to prospective patient’s place of residence
• Able to stoop and bend; must be able to lift and transfer patients
Desired:
• Experience with a public health, home care, home health care, or hospital background
• Ability to demonstrate communication, presentation, interpersonal, analytical and problem-solving skills
• Bilingual
